Across Boundaries: An Ethnoracial Mental Health Centre

Across Boundaries provides services such as case management and holistic programming using an “anti-oppression “  framework, in over 25 languages for the At Home/Chez Soi project. The Toronto project will include a focus on evaluating this type of case management service as it applies to the target homeless population.

Centre for Research on Inner City Health

The Centre for Research on Inner City Health is responsible for program evaluation and measurement of the At Home/Chez Soi project. The trans-disciplinary centre is focussed on reducing health inequities through innovative research that supports social change. CRICH is part of the Li Ka Shing Knowledge Institute at St. Michael’s.

City of Toronto Shelter Support and Housing Administration

This arm of the City of Toronto is responsible for housing coordination and administration and the implementation of the “people with lived experience” caucus for the At Home/Chez Soi project. Shelter Support and Housing Administration is the service system manager for housing and homelessness programming in Toronto, including social housing shelters, housing help services, street outreach and drop-in centres.

COTA Health

COTA Health is responsible for providing Assertive Community Treatment (ACT) services to 100 people with high service needs within the At Home/Chez Soi project. COTA Health is an accredited provider of mental health and community support services for people living with a mental health challenge, geriatric mental health condition, developmental disability, dual diagnosis, acquired brain injury and/or the experience of homelessness. Services include case management supportive housing, short-term residential beds, day programs, court and justice related services and outreach to people who are homeless or living in shelters.

Toronto North Support Services

Toronto North Support Services provides case management services to 100 participants with moderate service needs in the At Home/Chez Soi project. Toronto North Support Services is a community-based mental health and homeless services organization that provides a range of programs including street outreach, follow-up support, case management and housing supports.
